Log into your account at <a href="http://app.gimme.care" rel="nofollow noopener noreferrer" target="_blank">app.gimme.care</a>.

Click your profile icon in the top right corner.

Select Manage account, then go to the PHI tab.

Under Protected Healthcare Information, click Download data to save a copy, or View data to review it online first.
